Description: This geriatric medication management webinar will offer review of the case studies of older adults who experience adverse effects due to inadequate medication management during care transitions. With the Beer’s criteria as a guide, medication pharmacology, dosing, adverse effects, and management will be discussed. Poor outcomes related to omission/duplication, dosing errors, drug-drug interactions, and inappropriate prescribing/deprescribing will be revealed. Key contributing factors to these outcomes and measures to prevent them will be highlighted.

Carrie Bennett is a board-certified Geriatrics Clinical Nurse Specialist at UnityPoint Health -Meriter Hospital in Madison, Wisconsin and a Clinical Associate Professor in the DNP program at the University of Wisconsin-Madison School of Nursing. Her clinical interests include fall prevention, dementia, and delirium. In her CNS role, she works closely with patient care teams to improve the outcomes of hospitalized older adults, paying particular attention to prevention and management of geriatric syndromes. In her Clinical Associate Professor role she functions as the Population Lead for the DNP-CNS track and teaches courses in leadership and quality improvement as well as the DNP scholarly project courses. In her spare time she enjoys cooking, baking, and spending time with her family.
