2027 CNSI Fellow Program Application

  • Highest degree, licensure, certification, other (e.g., MSN, RN, AG-CNS…).
  • Applicants must be current members of NACNS and have at least two (2) consecutive years of membership immediately prior to application.
    Applicants must have five (5) or more years of experience working in a Clinical Nurse Specialist (CNS) role after completing graduate education and training. I confirm I have five or more years of experience working in a CNS role.

      Submit two (2) letters of endorsement from professional colleagues; preferably one letter from a supervisor or individual in a position to evaluate the applicant's work; the second letter should be from another peer/colleague who is knowledgeable about the applicant’s work as a CNS. Endorsement letters should address the criteria (see criteria above in #2) and be no more than 2 pages in length (signed, typed on organizational letterhead). Endorsers may only endorse two (2) applicants per year

        Applicants must write a statement that provides evidence of their outstanding contributions as a CNS. The statement must address the criteria (listed in Application Instructions #2) and provide clear exemplars in support of the statement. Organize the statement using the criteria as the headings for the statement with the supporting evidence below each heading. 750 word maximum including headings.

          Write a concise (150 word) professional biography highlighting key accomplishments and significant milestones in your career. If the applicant is selected, this biography will be used in promotion materials. The biography should begin with the applicant’s name, current CNS role or title, and employer. Provide a summary the applicant’s outstanding contributions to the CNS practice, education, and/or leadership and include major honors and awards. 150 words maximum.

            Provide a curriculum vitae that documents evidence of CNS leadership, practice, research, dissemination, service, and/or educational activities in support of the evaluation criteria. The curriculum vitae must include the applicant’s name, credentials (highest degree, licensure, certification(s), designation), job title, and contact information (Ex. Cherry Aims,MSN, RN, AG-CNS). The CV must include separate sections with the following headings: 1) Education (list the most recent degree first, followed by prior degrees; include, institution, degree earned, and year awarded); 2) Employment History (include employer, position, and dates of employment; do not include job duties); 3) Honors and Awards (include award title, granting organization, and year received); 4) Research/EBP/QI Projects (include project title, applicant’s role, and, if applicable, funding amount and project duration); 5) Dissemination (including full citation of publications, book chapters, and selected presentations); 6) Service Activities (organized by local, state, national, and/or international levels; include committees, projects, professional activities, and leadership roles); 7) Legislative/Regulatory Activities (include activities that promote the CNS role at the local, state, or national level).
